I have never once had an issue where my Vanguard account let me in without requiring the one-time passcode. I had it set up to call me with the one-time passcode but now when I enter my username and password it lets me right into my account. It worked correctly for a while but then it just stopped. Has anyone had an issue with Fidelity's two-factor authentication (2FA) not working? I set 2FA up several months ago with Fidelity.
